Referral Process

A complete Client Referral Sheet and Release of Information for school records must be submitted to DVR by public school staff in order to be recorded and tracked.  Once the Client Referral sheet is received, student and/or family will be contacted by DVR staff to schedule an Initial Intake.  An informational packet will be mailed to the student/family to be completed and brought to the Initial Intake appointment.

Eligibility Determination Criteria

Must meet the following criteria to be eligible for DVR Services:

  1. Must have a diagnosed disability.  Proof of disability may include an Educational Diagnostic Evaluation within the last 3 years signed by the school diagnostician; Medical reports from an MD; or diagnosis from a licensed clinical therapist, social worker, psychologist or psychiatrist.
     
  2. Disability causes an impediment to employment.

Order of Selection

An Order of Selection is necessary when there are not enough resources to serve every eligible person.  In that case, DVR implements a waiting list.

Individualized Plan for Employment (IPE) Development

An IPE is a plan of services and activities that can reasonably be expected to lead to an employment outcome.  This may help determine the goals, objectives, nature and scope of vocational rehabilitation services which are to be included in the Individualized Education Plan (IEP).


Case Closure

After 90 days of successful employment related to IPE goal, DVR will close the case if the client agrees and is working in suitable employment. Post-Employment Plan services are offered once a case is closed successfully to assist with maintaining employment.  In some cases, participants may be asked to re-apply for vocational rehabilitation (VR) services, and a new eligibility determination is made.
